Becoming a new Patient
We are NOT currently taking new patients. We will ALWAYS accept NEWBORNS and their siblings.
New Patient Information
- We have a waiting list you can be placed on. We call each family on a first come, first served basis. If you do not respond to confirm continued interest in our practice within 7 days, we will skip to the next family in line.
- You may check the link below to see if providers are taking new patients. You may also call the office to check your place in line on the waiting list
Newborns
- Please call the office to be placed in our "Baby Book" prior to delivery, so we can anticipate your new arrival.
- You must call us from the hospital after delivery, but prior to going home, so we can set up his/her first newborn visit.
- If you deliver on a weekend, Friday after 1 pm, Saturday or Sunday, call the following Monday to set up a visit.
Patient Forms
- Before your child's first appointment with us, please print out, review and sign the required clinic forms. Bring the signed forms to your first appointment. If you are unable to print the forms out, you can fill them out when you arrive for your appointment. Please arrive 15 minutes before your scheduled appointment to fill out all remaining forms.
